Early Intervention Change Ideas

Each change idea link below includes a PDF with step-by-step guidance for pediatric practices. The related sample tools (e.g., checklists, scripts) can be edited and customized.

  1. Offer brief intervention from an early childhood developmental expert or team member to provide immediate guidance and support.
  1. Establish a secure process for communicating with external specialists and community-based organizations to improve care continuity and follow-up documentation.
  1. Introduce a standardized operating procedure for early childhood telehealth visits to improve consistency, efficiency, and developmental outcomes.
  1. Use community health workers or patient navigators to support telehealth visits.
  1. Integrate home visitor drop-ins during well-child visits to strengthen coordination and personalized care.
  1. Establish a collaborative care model to coordinate primary care and specialized child health and behavioral health services.
