This session highlights the Model Convening Project – a partnership of four complementary, evidence-based models – and its Hand in Hand framework, a framework for community collaboration that achieves positive, equitable outcomes for all young children and their families. Dr. Paul Dworkin, founding director of the Help Me Grow National Center, moderated the session. Presenters included leaders from the four Model Convening Project partners: Family Connects International, HealthySteps (a program of ZERO TO THREE), the Help Me Grow National Center, and the Nurse-Family Partnership.
