This video explores practical strategies for strengthening team-based care to better meet the developmental, health, and social needs of young children and their families. Drawing on focus groups conducted with pediatric practices through the Transforming Pediatrics for Early Childhood (TPEC) program, the video highlights the benefits of a team-based approach, including more coordinated care, stronger relationships with families, clearer roles across the care team, and improved staff well-being.

The video shares approaches that pediatric practices can adapt to their own settings, including purposeful scheduling, expanding or adapting team roles, strengthening communication, and using technology to support coordination. Pediatric practices, as well as the programs that support them, can use these strategies to take practical steps toward more connected, comprehensive care for children and families.
