This screening passport, available in both English and Spanish, can be used by parents and caregivers to track their child’s screening history and results.

Developmental screening is a way to check a child’s development and determine if they are on track or might benefit from extra help. Screenings are recommended for all young children and are administered by a doctor, teacher, child care provider, home visitor, or other professional. This screening passport, available in both English and Spanish, can be used by parents and caregivers to keep track of their children’s screening history and results. Parents and caregivers can use the screening passport in several ways, such as filling it out whenever their children have a development screen, taking the screening record to each check-up with their children’s doctor, and talking about screening results with their doctor.
